Organization of Your Health Profile.

You, as the patient, are the best gatekeeper of your health record! Electronic health records (EHRs) can’t always share your data from one healthcare provider to another. Our advocates will listen and gather your health history and compile it in an easy to navigate record that you can share with every healthcare provider you see. Having an accurate health profile and database, leads to less duplication of tests and medical errors.

Care Coordination & Transition of Care.

Managing chronic diseases or figuring out next steps after a new diagnosis can be daunting and overwhelming! Our Patient Advocates can help you step by step understand your disease states and what your healthcare team is recommending. The time you spend with a Patient Advocate discussing your questions and concerns, will help you more effectively communicate your healthcare issues and desires with members of your healthcare team.

A hospital stay is never easy and coming home sometimes is scarier! Our Advocate team can help you understand your discharge plan and help you facilitate any at home care/services you may need.

Health Education.

Do you have goals for your health and don’t know where to start?

Our team of Advocates have years of collective experience helping patients reach their desired health and wellness goals. When you engage with one of our Patient Advocates, they will help and support you in creating your personal health and wellness goals. We treat each patient as an individual and meet you where you are in your health and wellness journey!

Pre/Post Surgery Navigation.

Are you scheduled for an up-coming surgery?

Going through surgery can be a fearful experience. We are here to help answer your questions before and after surgery. Our Patient Advocate will review your Individualized Health Profile and Database making sure every member involved in your surgery understands your health history, and will help you understand the steps you need to take prior to and after surgery to ensure a successful recovery.

Medication Overview.

Are you struggling with prescription costs or don’t understand what or why you’re taking some of your medications?

Managing and understanding medications can feel like a fulltime job! Our Patient Advocates can help you locate resources to help make your medications affordable, review your medications with you and help you understand why/how your medications might be helping to improve your health and lower certain health risks, and answer any questions you might have regarding medication management.

Insurance/Billing Navigation.

Do you have questions about insurance or a recent healthcare bill?

Health insurance and billing is complicated! Your Patient Advocate can help you choose the insurance plan that will best meet your healthcare needs and can also help you understand and negotiate your healthcare bills.

Employee Wellness.

Why invest in employee wellness?

The value of investing in the health and well-being of employees has been well researched and documented with studies stating return on investments (ROIs) range from $3-$6 for every $1 spent on wellness programs, depending on how robust your employee wellness plan is. Healthy and happy employees also leads to less absenteeism, improved productivity, retention and can help lower your overall employee health insurance costs. Advocate LLC’s Employee Wellness Plan includes two health screening events at your place of business. At these events, employees will be screened for high blood pressure, diabetes and cholesterol disorders. After each event, employees will then get a 30-minute appointment with one of our Patient Advocates to review their results and help them create a personal health goal. In addition, you and your employees get a 10% discount on all of Advocate LLC’s services and exclusive access to Healthy Values TeleTriage Acute Care.

Health insurance and billing is complicated! Our Advocates help self-funded employees identify providers, facilities, and pharmacies with transparent pricing, empowering them to
make informed healthcare decisions. They can also assist employees in negotiating individual healthcare bills, helping to reduce costs and stress.

Our consultation services are offered through a retainer model.  This flexible approach ensures you have dedicated access to our patient advocacy expertise as needed to support your team's health and wellness needs.
